Device to detect a faulty transformer

I am trying to build a Device that can detect if an electrical transformer is faulty. It should be a small and portable. I suspect this device would be attached to the GnP fuse and also have a connection with a control room to detect if the reason for no Power to the G and P is becuase of lack of electricity supply.

 

1.Device Spec: 2.Waterproof 3.Durable 4.Should have a sim card slot so that a signal can be sent via GPRS if transformer is found faulty 5.Easy to install

 

I woudl like to get suggestions and advice and know who can join me in building this.
